The term broken shoulder normally refers to a fracture in either the proximal humerus (the top of the upper arm bone), or the scapula (shoulder blade). The clavicle (collar bone) in the third major bone that makes up the shoulder joint, and may also be fractured. The humerus and scapula bones are attached in a ball and socket arrangement. This allows the shoulder a great range of flexibility and mobility, while at the same time allowing it to bear considerable strain and pressure. The shoulder blade itself is a difficult bone to break, requiring a high force of impact, and fractures to the more fragile humerus bone therefore make up the vast majority of broken shoulder injuries.

Around 80% of shoulder fractures are non-displaced, meaning the bones will realign after a period in a sling, without the need for surgery. A displaced fracture is one where the bones have fragmented or separated, and will not realign without surgical intervention. In such cases, pins and screws may be inserted to hold the bones place. Shoulder surgery is complicated by the number of muscles, tendons, ligaments, nerves and blood vessels that surround and support the joint. The shoulder joint is a fragile structure, and permanent damage may cause a serious disability. In these circumstances, the only option may be artificial shoulder joint replacement surgery.

When a person falls sideways off a platform, such as a roof, ladder, or scaffold, the brunt of the impact when they land may be borne by the shoulder or hands.
